In order for you to receive the most benefit from your appointment, it is important that you arrive on time. If you are a new client please arrive 10 minutes before the scheduled appointment time to complete a new client information sheet. You can also complete the new client information sheet by downloading the PDF. Print, fill out and bring it in to your first session.

Please call me if you have questions regarding your treatment, your progress, or any concerns. If I am not available, you can leave a voice mail message include the best method and time to reach you. Your call will be returned promptly.

Policies:

Cancelation policy:
24 hour advance notice to cancel any appointment is required. For example, if you have a 9am appointment on Tuesday you must call before 9am on Monday to avoid being charged. Failure to give notice of your inability to attend an appointment will result in being billed the full amount for the session.

Each client will be allowed one cancellation with less than 24 hours notice without being billed. After one late cancellation or no-show the client will be billed.

Late arrival:
Clients who arrive less that 15 minutes late will receive the remainder of their session, which will end at the scheduled time. The full price for the appointment will be charged. It is at the therapist's discretion whether or not to accept a client who arrives more than 15 minutes late.
